WHO is ELDERHELP?

ELDERHELP fills a need for care management of older or disabled adults
under the direction of Kay Friedlander who has worked with those having
a variety of disabilities including stroke, Alzheimer's, mental illness
and persons who may be physically or mentally challenged.
Kay Friedlander, qualified by education and experience, is a NY Certified
Social Worker, licensed in PA as well. She received her Master of Social
Work from Syracuse University and her Bachelor of Social Work from Cornell
University. She is certified in Gerontology and with her broad range of
experience has a unique understanding of the psychosocial issues affecting
older adults and caregivers.
A member of the National Association of Professional Geriatric Care
Managers, the National Association of Social Workers and the American Society
on Aging she is also a Certified Court Evaluator, accredited and approved
to assess the needs of individuals in guardianship proceedings in the Courts
of New York State.
WHY use a Professional Care Manager?
You have serious concerns about a frail or disabled
adult and need information and assistance to ensure safety, independence
and quality of life
You have no family living nearby. Our care manager
is here to help. Not as a replacement for family but as an extra member
upon whom you can rely to advocate for you. A professional, who knows how
to determine needs and link you to appropriate services to make for peace
of mind.
WHAT is private care management?
ELDERHELP professionally assesses the unique needs,
abilities and sources of support of an aged or disabled adult. Physical,
emotional, psychological, legal and financial factors are weighed to establish
a context for appropriate care.
ELDERHELP develops an individualized care plan,
providing for immediate support and long term needs. Care management may
locate and arrange for services, screen and assist in hiring providers for
home care, financial and legal services, home maintenance, meals, transportation,
pet care and other services. Care managers may accompany clients to medical
appointments, will coordinate services, monitor health and other care providers,
periodically reassess the client, and are available for telephone reassurance.
All this in the effort to maximize independence and autonomy and make it
possible for adults to stay at the least restrictive level of care, often
in their own homes.
ELDERHELP will assume responsibility for care
management or share responsibility with family. Our goal is the creation
of a living environment that is safe and supportive.
WHEN is ELDERHELP needed?
ELDERHELP care management is needed when an adult
is faced with decreased capacity to function due to complications of aging,
illness, disability, altered cognitive status and/or a loss of social support
systems.
ELDERHELP assists when time and stress in providing
care for a family member may conflict with the caregiver's work, family
or other obligations. Care management can be an essential link in the chain
to assist long distance caregivers.
Short-term counseling for clients and caregivers
may be needed to assist in decision making and problem solving of life issues,
such as: changes in physical and mental status, chronic illness, grief and
loss, retirement, widowhood, relocation, family conflict, care for a family
member, long term care. A call to ELDERHELP will ease the burden and provide
peace of mind for caregivers. Care managers are available after-hours for
consultation.

ELDERHELP care managers will monitor and advocate
to ensure that needs continue to be met by selected service providers.
HOW does one pay for ELDERHELP?
ELDERHELP associates, inc. private care management
is provided on a fee for service basis. There is an initial assessment fee
and an hourly rate for care management services. Fees are stated in written
form and approved by the person accepting responsibility for payment prior
to initiation of services.
ELDERHELP strives to eliminate or postpone inappropriate
institutional care, and to utilize local services and government entitlements,
thereby reducing costs and effecting client savings. Time and money can
be saved for the long distance caregiver who must forego work and family
to coordinate care for a loved one.
