Welcome! We have prepared this page to help answer the basic questions you may have about our practice. If you have additional questions, do not hesitate to ask anyone in our office.
Ours is a family-centered practice, and we welcome the opportunity to care for as many of your immediate family as you wish. We believe that family-centered care results in a more intimate and practical knowledge of your family's health and your individual needs.
We strongly believe in prevention first and in healthy life habits, and we emphasize these wellness concepts in our practice. We urge you to choose one of us as your primary care provider and to see that provider whenever possible so that a thorough knowledge of your health needs may be acquired. When it is not possible for you to see your provider, another one of us will be available.
New patients are asked to have a complete physical examination
within three months so that we can review your health status with you and
provide you with the best in preventive care.
We care for our patients from birth through childhood, adulthood, and the later years. We do office procedures, minor surgery, and handle injuries such as lacerations. We have a very active obstetric and pediatric practice.
We emphasize patient education and patient involvement in health care by taking the time to listen to our patients. Preventive health advice and support is a focus. A coherent health plan is outlined in our preventive screening handout. It explains how frequently children and adults should be seen for preventive checkups and details the frequency of certain laboratory and diagnostic tests and the most important health habits you should follow. Please ask for a copy of this pamphlet.
Soon after becoming our patient, you should have a complete examination,
following which we will discuss a personalized health maintenance program
Our patient literature file, as well as audiovisual materials, are available to you. These are being continually updated.
Among the diagnostic procedures we are equipped to do are the following: electrocardiograms (which we interpret), X-rays on modern equipment which is monitored yearly by New York State, spirometry (an evaluation of lung function), pap smears and colposcopy to check the cervix for cancer, and flexible sigmoidoscopy to detect bowel cancer and other colon diseases.
We perform urinalyses, urine cultures, throat cultures, hemoglobin tests for detection of anemia, blood glucose tests, pregnancy tests, stool occult blood tests and other selected tests. Most blood chemistries are picked up daily by courier and taken to an outside lab which reports findings, usually the next day, by computer (The outside lab will bill you directly for its services. Some insurance policies cover these laboratory fees).
At our discretion we will make house calls for patients unable to come to the office. However, we generally feel we can offer you better care at the office.
We admit our patients to Cayuga Medical Center, where Drs. Klepack and Silcoff have general medical, pediatric and obstetrical privileges, as well as privileges in Intensive Care. Dr. Ferger has privileges in obstetrics and new-born care. If you prefer, we have working arrangements with doctors at the Cortland Memorial Hospital to admit and follow you there. We can also arrange for you to be admitted to any other hospital of your choice.
Your primary care provider at the office may not always be the one caring for you in the hospital. Hospital duty is rotated among colleagues whom we have chosen for their skill and care. You may be cared for by more than one doctor if you are in the hospital for several days - which is a common practice among most staff members at the hospital. We will resume your care in our office after your discharge from any hospital.
Please inform us of any emergency or family problems. We have facilities for minor surgical procedures and you may save time and money by seeing us for many emergencies in the office instead of going to the Emergency Room or the Convenient Care Center. Please call us if possible before coming to the office. If the situation is obviously critical or life threatening, go directly to the hospital emergency room and we will be informed.
Office visits are by appointment only except for emergencies. We expect at least 24 hours notice for cancellations. We reserve the right to bill you for appointments not cancelled in a timely manner. When you call for an appointment, our receptionist will ask a few questions regarding the nature and urgency of your problem or concern. Our receptionist needs to know why you are making an appointment so that she may allow enough time for your visit. Please be candid with her and be sure to tell her all the needs you have so she can allow enough time. Our staff members are chosen for their professionalism in matters of confidentiality, so you may be open with them. We do try to schedule your appointments with your primary provider (see above). Although we try to be realistic in our scheduling, we may get behind. Our receptionist will try to keep you informed.
For routine health care, please call several days in advance. Emergencies and problems requiring immediate physician attention will be given priority. Notify us before coming to the office in an urgent or emergency situation.
Appointments are scheduled as indicated. Saturday morning office hours are available for your convenience. Nursing personnel are available for routine laboratory work and tests from 8:30 AM to 5:00 PM Monday through Friday.

We gladly will share the joys of caring for your new infant by attending to its care at the newborn nursery at the Ithaca Hospital. We have full admitting and care privileges for the newborn nursery and pediatric ward and will consider it a privilege and pleasure to help care for your expanding family.
We offer you a wide range of services in office surgery and skin care. Removal of moles, skin cancers, cysts, warts, and repair of lacerations are among the procedures we do in the office. Freezing of skin lesions (cryocautery) is available.
We are able to meet nearly all your health needs; but in situations where referral to a specialist is desirable we will be pleased to help you select a well qualified specialist if you want us to do so. When you conclude your treatment, the consulting physician will send us all necessary medical information for your records.
We are committed to providing you with the best possible medical care, regardless of whether or not you have health insurance. In order to achieve this goal, we need your assistance and your understanding of our payment policy.
The fees that we charge are within the usual range for our area and specialty.
Payment for services is due at the time services are rendered, unless we participate with your insurance. Your co-pay, any deductible, or any amount not covered by your insurance is due at the time of your visit. We accept cash, checks, Mastercard and Visa.
We will gladly discuss any questions you have in regard to our billing and your insurance. However you must realize that your insurance is a contract between you, your employer, and the insurance company. You are ultimately responsible for your bill, for deductibles, co-pays and services not covered by your insurance. If your insurance requires a deductible to be met, it is your responsibility to know what that deductible is and whether or not you have met the deductible for that year.
We realize that temporary financial problems arise that may affect timely payment of your account. If such problems arise, we encourage you to contact us promptly for assistance in the management of your account. We are here to help you.
Electronic Medical Records
As we try to always implement new ways to improve the care we give you and the efficiency with which we care for you we have decided to use the newest in electronic record keeping technology.
We think you will see the following:
- Faster refills on prescriptions with our orders being faxed directly to your pharmacy in typewritten form (no more doctor's handwriting!)
- New and better ways to see if you are up to date with preventive health care.
- Even easier and faster scheduling of appointments and consultations with specialists.
- Fewer questions from insurance companies to you due to more sophisticated claims submissions.
- More information that our ladies at the front desk can provide to you about your outstanding account with us.
- And, as always, respect for your privacy and security for your medical information.
You will notice that we and our staff will be entering your visits directly into a computer not on paper as in the past. We hope this will not be distracting to you as we see you in the examination room. We will be glad to show you how it all works.
No prescriptions for Over-the-Counter ("OTC") medications
We have been notified that Flex accounts (also known as "cafeteria plans")
and Health Savings Accounts ("HSA's") will require prescriptions in order
for you to be reimbursed for over-the-counter medication purchases starting
January 1, 2011.
Due to the volume of prescription medication that we must already handle every day, and due to the fact that we do not recommend many of the supplements which are sold as OTC medications, we have decided that we will not be able to write prescriptions for over-the-counter medications. The only exception will be the few OTC medications which our practitioners recommend you take (e.g. calcium, vitamin D, aspirin for heart disease, etc). We will write prescriptions for these items if you need one in order to be reimbursed from your Flex or Health Savings Account.
Please do not ask us to write prescriptions for other items. Thank you for your understanding
