For years now we have provided our patients with forms to complete which will give them the power to express their wishes in the final days of their lives. We have urged all our patients who are of legal majority to complete these forms since one of the most common situations in which they may be of help is a motor vehicle accident. While strokes and serious medical illnesses are more common the older we get motor vehicle accidents can happen at any age.

Please take the time to access this web site, read the information, complete a proxy form and then give us a copy (as well as a copy for your proxy and yourself).

Remember to specifically write in on the form that "I have discussed with and hereby give my proxy the power to decide about artificial hydration and nutrition." - if you do not want to be kept alive by intravenous fluids and feeding tubes.  If your proxy decides that artificial hydration and nutrition would not be in your best interest, the proxy needs this direct statement on the proxy form in order to have the authority to refuse their use by the doctors and hospitals.
